New Patients
To schedule an appointment please call us during regular business hours at 317-842-5555 or use our online appointment request form below. Plan to arrive 30 to 40 minutes before your appointment to fill out all necessary paperwork. To fill out the paperwork ahead of time, click here.

Please call to notify the office if you are unable to make your scheduled appointment. Failure to show up for your scheduled appointment will result in a $50 charge.

What to Expect at Your First Appointment
Please bring in all your medicines, either in their original containers, or make a list of all medication you are taking along with the dosage and frequency. Include both prescription and over the counter medicines.

If using insurance, bring your health insurance card, a photo ID as well as any other insurance information you may need, to your appointment.

Make a list of all your health questions. Put the questions that are most important at the top of the list. Make a list of the mental health providers you have visited. Write down their names, addresses, phone numbers and the reasons you visited them.

We accept all major credit cards and personal checks. Please bring personal identification and insurance information to your office visit, and we will notify you if there are changes with the types of insurance we accept.
