Schedule Your First Appointment
To schedule your initial visit, please call us during regular business hours at 317-842-5555 or use our convenient online appointment request form.

Once your appointment is scheduled, please complete all required new patient paperwork prior to your visit to ensure a smooth check-in process.

If you cannot attend your scheduled appointment, please call to notify us. A $50 no-show fee will apply for missed appointments without notice.

What to Expect at Your First Appointment

To help us provide you with the best care, please bring the following:

All medications in their original containers, or a complete list with names, dosages, and frequency, including over-the-counter medications.
Your health insurance card, photo ID, and any additional insurance documents.
A list of any previous or current mental health providers, including their names, contact information, and the reason for visits.
A written list of your questions or concerns, with your top priorities listed first.

Payment & Insurance

For your convenience, we accept credit and debit card HSA card payments only.
Payment is due at the time of service. We do not accept cash, checks, or money orders.

Please bring your photo ID and insurance information to each visit.

If you have any questions regarding payment or billing, please contact our office and our staff will be happy to assist you.
